- New Patients: Complete the form below to schedule an assessment. Your cost is $290 and valid for six months. Payment details will be discussed during our call. Existing patients can order a Vital Care Box directly via call or email. The cost for a box is $260 each.
- Scheduling Call: Once we receive your completed form below, our team will contact you within 24 hours to arrange your 20-minute assessment call.
- Assessment: During the call, we'll review your medical history and health needs to ensure the contents of your Vital Care Box are safe and suitable for you, taking into account any allergies or interactions with existing medications. We'll also answer any questions you have.
- Delivery: Your tailored Vital Care Box, priced at $260 with included ground shipping, will be sent directly to your home. Instructions will be provided in the box. For an extra $30, overnight shipping is available.
- Follow-Up Care: Our team is committed to supporting you in your recovery journey, ensuring you're back to feeling like yourself. We offer ongoing support and medication refills for up to six months.

Overnight shipping will arrive the next if ordered by 12:00 pm. The price for overnight shipping has an additional charge of $30, making the Vital Care Box cost a total of $290. -
